You can make a raised bed as elaborate or as simple as you like. You can use wooden sides (held in place with brackets or stakes) or stones or cinder blocks, a prefab kit, or simply rake the soil into a flat topped steep sided "mesa". It may be anywhere from a few inches to a foot high.

The soil beneath the bed would be loosened and organic matter would be added, perhaps also a small amount of sand if the soil is heavy. Additional amendments would be added per the results of soil tests.

Loosening the soil will fluff it up somewhat, the amendments will also help raise it. The soil would be maintained over time by regular additions of more organic matter and the use of an organic mulch that would rot down over time.
