Clay Soil - Knowledgebase Question

I planted grass this past spring and early just after the planting, the soil that was added washed away. The lawn came in and there are bare spots in it. The soil is mostly clay. What can I add to the lawn this fall that will loosen the clay both on the bare spots and on the lawn? I was told that lime, peat, and certain types of fertilizers can help.
