how deep & how wide should the hole be dug

Robert,

I'm assuming we're talking about trees, shrubs or woody ornamental vine. If this is not correct please submit another question to the website.

Dig a hole as deep as the rootball and about twice as wide. That way you can easily plant the shrub or tree at the same depth it was growing before. If you dig the hole too deep and then fill it in the soil will settle in time and the plant will end up too deep.
