I'm looking for a 10' x 3 to 4' wide evergreen shrub or dwarf tree to plant a screen to hide a hot tub.

Columnar junipers are good choices as is 'Sky Pencil' holly. You might also try Thuja (American Arborvitae 'Pyramidalis'). This shrub grows tall yet remains only 2-3' wide at maturity. All are evergreen.
