Answer: Since you are transplanting the carnations you can simply knock off most of the old potting soil and replant them in fresh potting soil to remove much of the acid fertilizer. A little acidity won't harm them at all. Expect them to droop a bit until they adjust to their new homes. If the worms are earthworms or red wigglers, you can transplant them without problem. If they are short and fat they are grubs and should be destroyed.
