I move creeping phlox in bloom. Its pretty easy to transplant. As it is a running sort of plant it roots easily. I just dig it and divide. When I replant I look for the part that is coarse and almost woody. I bury that part. I also will put dirt over a bit more. Keep it watered and it should take quickly.
