You may not be doing anything wrong. If the heads are misshapen it could be the result of climatic conditions, root damage from nematodes, or even early season attacks by aphids. When you replant lettuce next year try growing it in a different garden spot to avoid soil dwelling insect pests, keep the soil evenly moist, and provide about 5 hours of direct sunshine.
