Tree 5 years old has not bloomed-- died back 2nd year and shot up from bottom. Why has it not
bloomed?

In my experience redbud trees often fail to bloom well in the year or two after transplanting and younger trees occasionally take a number of years before they bloom at all. It may be that the combination of planting and die back stressed your tree so that it is simply being "shy" about blooming. As long as it seems to be growing normally, I would be patient. Maybe next year will be it!
