Would it hurt an apple tree with a hole (about 3-4") to be filled with the expanding foam that you use around windows and doors?

Unfortunately, such a hole is often an indication of more serious problems within the structure of the tree, problems not obvious to the untrained eye. I would strongly suggest you consult with a professionally trained arborist about the health, stability and care of the tree.
