Do I cut all the old canes from the previous season's growth to the ground or not?

Hydrangeas can be pruned down to 12-18" from the soil. You'll want to leave at least two buds on each cane so that the new wood produced this spring will develop flowers. If you prune all the way down to the ground, this year's growth will not flower.
