how do I get rid of the grubs and the mole holes in my yard once and for all.

If the grub population is high enough to create a problem in your lawn, you can apply beneficial nematodes (such as Grub-Away) to control them. Read and follow the label directions for how and when to apply. Here is more information about beneficial nematodes you may find helpful.

Moles are an indication of healthy soil and eat an assortment of insects, slugs, and so on, they are not a problem in and of themselves. (Moles do not eat plants.) If their tunnels are creating a safety hazard, the most effective control is to trap them. Here is information on that.
