This my be a silly question does the tomatoe plant grow up spouting out from the top or push up from the bottom this is concern tying the string to stake and cage.

Not a silly question at all! Tomato plants grow stems at the top of the plant but also grow new stems on existing stems that are already attached to the main stem at the bottom of the plant. It will bushy all over but most of the new growth will be at the top of the plant and the ends of the branches. If you are staking or caging your plant and need to tie it, place the ties loosely along the largest, most upright stems about midway along the plant. As new growth appears at the top and sides of the plant, you can loosely tie those stems, as well. Hope this clarifies things for you!
