Overwatering or poorly draining soils are usually the culprits when it comes to root rot with cactus. You may be able to save your cactus by digging around the root system and cutting out the moldy roots with a sharp knife, then filling in the hole with sand or a gravel/sand mixture. Be sure to be extra careful in watering. I water mine about every 2-3 weeks during the summertime and once every 4-6 weeks in spring and fall. Maybe this schedule will work for you, as well. Hope so!
