What can to done to prevent or kill worms on tomato plants?

Small green fruit worms, and big green hornworms can be hand picked from your plants or you can use bacillus thuringensis (Bt) which is sold as Dipel or Thuricide. Bt acts as a stomach poison to caterpillars but is not effective against other garden pests. Be sure to apply according to label directions.
