Please tell me the name of the fertilizer and when & how much I should apply.

You can use a general purpose garden fertilizer (10-10-10), scattered beneath the plant at the drip line and watered in thoroughly or you can simply use an organic mulch at the base of the plant (compost or aged manure). Use the fertilize package recommendations which will vary depending upon the size of your pieris.
