The worms are the larvae of the codling moth and you'll need to spray your tree on a regular basis all summer long to keep the fruit safe from these pests. Permethrin and Malathion are registered for use in pear trees. Look for either of these as the main ingredient in a fruit tree spray and apply according to label directions. Best wishes with your pears!
