Bought tree 4 years ago. Granted, it does get wet from neighbor's drain, but flowers nicely, just very few leaves anytime. Now a few are red! Fertilized w/ high nitrogren to encourage leaves after flowers fell, even threw in some acid loving fertilizer too. No improvement. Next???

Sounds like you've done almost everything possible to encourage healthy new growth. About the only thing you haven't done is prune your serviceberry back a little. Pruning encourages new growth so try pruning. If the roots are healthy the serviceberry will respond to pruning with healthy new growth. I'll keep my fingers crossed for you.
