Neem oil was recommended to me for fungus, insects and caterpillar control. I have sprayed my Butterfly Weed twice with it and the caterpillars are still eating away.

I'd try Dipel or Thuricide for caterpillar control. It is a bacterium that acts as a stomach poison and specifically targets caterpillars. Of course you know that butterfly weeds attract butterflies and they are likely to lay their eggs on the plant so the newly hatching larvae (caterpillars) will have something to feed upon. The caterpillars you kill are the next generation of butterflies. I'd just leave the caterpillars alone. Your plant will recover when the caterpillars stop feeding and pupate.
