I found this seed in my apple and was wondering of it's already germinated. Can I plant it directly outside?

Welcome!

Yes, its germinating. You can plant it and it will grow (unless you damaged it taking it out of your apple). Remember where you put it.

Do I plant the seed sprout-side down or up, or does it not matter?

The little sprount is a root but, just plant it on its side. That's how they land when they're inside a rotting apple.

This is a good article (link below) from an agricultural extension source about seed-raised fruit trees. Remember, your tree will have a mix of characteristics from both it's two parents, which may be different from the apple you ate!
But isn't that the fun of growing it?

By the time this tree grows up and has apples, cchen1628 will have forgotten what apple they ate.
