Plant ID forum→Trying to figure out what this is before it dies!

For some reason, this plant has not been doing well in my front garden for the last couple of years. I'm trying to figure out what it is so that I can help it. It's leaves are turning a motled yellow and half of the tree didn't survive the winter :(
