Lavender was perfectly healthy, but now all the heads are falling over! Am I over/under-watering it? Is it rot?

Hard to tell. What you have is a Spanish, or French lavender. Since these like to be kept on the dry side, I suspect overwatering. Does the pot have good drainage? When did you last water? I would suggest to cut back on watering and see if it revives.
Yesterday is gone. Tomorrow has not yet come. We have only today. Let us begin.
