Pests and Diseases forum: Maple tree crown dying out and small fungus on a branch

We have a maple tree in our front yard. I'm not sure what kind of maple it is. Anyways, for the past few years the top of the tree has not been growing in. I've neglected to do anything about it until this winter during a couple of storms some of the branches began to fall off the tree. Thankfully they didn't hit our cars. Anyways, I'm wondering what might be causing the crown to die out and if I can treat it. Can I just got off the dead branches and hope for the best? Here are some pictures.
