Looks like mold, mildew, or rot. Take a Q-tip, dip in alcohol, see if it wipes off, and if spots on cati are soft/mushy.
Get those rocks off of soil, so soil can dry out faster. Let soil dry completely before watering.

Hi, I just did that and the cactus is super soft, almost like a deflated basketball or something. When I touched it to remove the spots I accedently punched a small hole though it Sad does it mean it has too much water? I'm confused I only give it a bit of water on the 1st of each month and spray her a bit on the 15th.

Sorry 😞, my friend.
Time for eulogy, and proper burial.
Don't give up though 👍
I killed many, before I got it right. Especially those fat kind.
One ! Just one ! Overwatering ! Pow ! Root Rot city. 😭😭😭

You want drain holes in pot, and no rocks/gravel on top, fast draining soil, so much to go into.
Please Lovey dubby check on care of particular variety/s .
Get back hear ! Same post.
When you get new one, buy a tad bit bigger pot, for it. also, equal size bags of cati soil mix, and perlite.
I'll guide you.

Ttfn friend 😀

I agree with Philip that your cactus is dead - and it may have been dead when you got it. When you look for a new cactus, avoid glazed pots without drainage and glued in gravel. Look for a cactus with a plumb, bright green body. If you are unsure, buy from a local nursery with a good reputation - avoid the Big Box stores.

BTW, the pot should only be about 1/2 inch wider all the way around then the cactus is, so that pot is actually too big.
Life is not a journey to the grave with the intention of arriving safely in a pretty and well preserved body, but rather to skid in broadside, thoroughly used up, totally worn out, and proclaiming...."WOW What a Ride!!" -Mark Frost
