What should i do to fix? I usually water when the top feels dry or when my moisture meter reads 4 (just before dry reading) usually about once a week.

Stop using the moisture meter because it may be misleading you. I cannot see the pot and how it is planted so I can't tell you how to improve the watering, although I don't think you are far off.
