There are a number of issues, but maybe the most-troubling is the holes, yes? That's probably rose slugs. They're teeny teeny little caterpillars, and they live on the underside of the leaf, and eat away.

I look for them on the underside of the leaf, and squish them. !They POP! <G>

Now, that rose is in a pot. How big is the pot? If you're going to try to grow a full-sized rose in a pot, you need a BIG pot. A rose needs lots of room for its roots.
