My brother gave me this orchid to regenerate.

What's the yellow stuff? Looks like sponge?
Does medium look ok? Or new medium?
Leaves looks dehydrated.
Should it be potted up to a bigger pot? Had to cut the plastic plug holder out. Took off 3 yellowed leaves.
