Well, there is really not much to it. I just went out and dressed this one up a bit. I used and old flag pole as a handle, I removed the duct tape and drilled a hole in the bottom of the container, used a couple of washers and a screw to mount it with. I did just fertilize hundreds of daylilies with it just with the container attached with duct tape and had no problems.
The key is getting the shape and size of the holes in the lid matched to the fertilizer you are using so that it just takes a slight shake to get the amount you want to come out. I show in the photo a slot and a lid with some round holes. The holes may look too large but for the mixture of fertilizer I was using they were about right.

Of course the length of the handle and the size of the container are up to your personal preferences.

So tempting to do a political thing here but I will refrain.
You attach a container on a stick or pole.
You have a few different lids you can put different shaped and sized holes into.
You put fertilizer into the container, select one of the lids that look like the grain size for the fertilizer you are using.
Hold the handle with the container hanging down.
The fertilizer should come out in the amount you want with a shake or two. Proceed about the clump.
The point being, if you have hundreds of daylilies to fertilize this can save your back from all the bending.

It looks like your example is using a plastic peanut butter jar. I like that you can save all your peanut butter jar lids and poke different sized slots and holes in them to handle different types of fertilizer or snail bait, etc. Are you drilling pilot holes through the jar and pole, or just screwing it on there with a washer to keep it from popping off?

I love a good gadget. Seems like it will save fertilizer because you can hit the exact spots you want, as well as save my back. I'm going to finish off some peanut butter for lunch. Smiling

I drilled a pilot hole in the bottom right down the center, put a flat washer on both the inside and the outside of the plastic jar.
Edited to make it clear that there is just one screw through the bottom of the plastic jar holding it to the wood flag pole handle.
