Sorry I'm late to the party. The Phal. spotting looks like Phyllosticta Leaf Spot to me. This is generally not a serious problem but rather an ascetic one. Spray with Mancozeb to control it if it bothers you.

I think you are on the right track, Ted. It is a larvae of some kind. I don't think we have June bugs in Florida but surely we have 10 others to take its place. Bt. or something like it should do the job.
