Sharlene I actually have a nice pulldown date form in there, but the browser you are using (Internet Explorer 11) unfortunately does not support it.
(Insert rant here about how IE can never keep up with modern web development techniques).
You'll have to enter the date manually, or try using a more modern browser that actually works. I hate to ask you to switch browsers in order to get this functionality, but for now that's really the only option. Have you ever tried Chrome? It's very easy to install and works really, really well!
