The genus is actually Rosmarinus, so you'd enter Rosmarinus officinalis for the botanical name.

Chef's Choice is the trade name and 'Roman Beauty' is the cultivar name. Don't add the PP number to the cultivar or trade name. So your fields should be:

Primary common name: Rosemary
Accepted botanical name (Genus species): Rosmarinus officinalis
Cultivar: Roman Beauty
Trade Name: Chef’s Choice®
