I am very bad a record keeping but I do have a general strategy... much like Trish's.

I use Osmocote on lots of flowers in the ground and potted plants. Certainly my daylilies per Lyle's advice.
I use fish emulsion on the lawn... since my dog is known to eat grass. Organic.
I compost to supplement mostly container veggie growing and don't fertilize at all. I use purchased organic soil for those plantings.
I don't fertilized my very mature Arizona Sweet Orange or my Key Lime tree. They are PROLIFIC without any TLC at all. (except the hail storm of Oct 5, 2010 was very hard on them)
I use Miracle Gro on my Brug only... though it's near a couple of shrubs so I assume they reach for the nutrient.

But, I'd say all of this is by happenstance. I don't have a journal and can't remember anything. I fertilize when I think of it.

I have a friend who follows Martha Stewart like a religious guru. Martha says to keep a journal about fertilizing, pruning, thinning etc. Her garden doesn't look any better than mine. It's a temperament, organizational and control thingy. No? Big Grin
“Don't give up too quickly"... unknown, I heard it somewhere.
