Yes it is a Phalaenopsis.
They are typically sold at big box stores in those containers. From your image I see that it is growing in old sphagnum moss which is not good. As the moss breaks down, the roots start to die. In looking in the pot, there appear to be several compromised roots.
It could use a repot, either into new sphagnum which is cream and cinnamon colored OR into a bark mix of some type.
The pot that it should be in should have drainage holes in it so the potting material can drain properly. If it does not drain, water and moisture build up causing the roots to die back.
