I had a problem printing out my mom's form and Cathy at Scott's customer service was very helpful. She gave me a case number to include with the receipt since the form wouldn't print. (I haven't sent it in yet so I can't promise you that it works.) They might not give you the credit on the wrong product, but they may allow you to buy the right product for the rebate if you call and explain the situation. Since the product ends up being free or nearly so, it might be worth your time.
