The permit you need appears to be:
PPQ 587 - Application for permit to import plants or plant products
I tried doing the online registration...it is very involved to set up an online account and I will have to take my government issued ID to the local USDA office to verify who I am to activate my account. I may still do it because I can't print the form at home (broken printer)...but I'm thinking for anyone else it may be easier to print and mail the application...or call the phone number.
