To me your plant looks like some type of Echeveria..though it does seem to have jade-like leaves but the leaves seems more plump, so maybe echeveria or a pachyveria. So whichever type it is, I will care for that plant by making sure it does not sit in water & less watering especially during the cold months, south/east/west facing window for warmth. Your plant looks okay to me, you can also add some more perlite or pumice to make the soil more open and fast draining too.

That's common for Pachyphytum, fortunately, they root easily which it seems from your photo that you have some already rooted at the base.
