the bent-over appearance is caused by the plant using-up its stored resources from its caudex + main stem .....as a result, the main stem is loosing rigidity (from loss of resources).....and can not support the heavy top
the stem wrinkling is usually normal ....but in this case may be something else - stem rot .....
the leaf wrinkling is also normal and occurs during dormancy after watering has ceased
the plant is planted too deep (too low) in the pot ....judging from the height of the lowest 2 stems nearest the white rock...they should be an inch or so higher (more "empty" space between 2 stems and white rock) ...... from seed, adenium grow into a bottle-neck shape - the first leaves emerge just above the bottle-neck ..... the bottle-neck being the caudex growing above the potting media and should be visible ...
pour off as much as the white rock as possible ...then gently pull the plant from the potting mix, knock off or wash off all potting mix -
then - examine the roots carefully (be gentile not to break roots) ....look for any root rot ....rot can be black to brown - wet to slimy ...and may or may not have a rotten smell -
if all looks good - and no rot is found .....
repot plant, but this time, lift it higher in the pot than where it was -
watering might correct the stem wrinkling ....(return to "smooth and plump") ....but watering now might also cause the death of the plant .....
another course of action - prop-up the leaning top with a forked stick ... or a hand size rock ....or a short length of bamboo / cane ....and wait out until spring - when watering can resume
consider cutting off the main stem ....if that is stem rot ...and if left as is ....rot will travel throughout the plant - eventually killing it
