Start by raking out the bald spot and digging in a little compost to help loosen the soil. Then spread grass seed over the spot and keep it moist until the seeds germinate (usually 7-10 days). After the new grass is up and you've mowed the lawn 2-3 times the roots will be established enough that you can apply a weed killer. You can either use a weed and feed product to help control the weeds or you can feed with a Turf Builder fertilizer and spot treat the weeds. Keep your lawn watered and mowed regularly and it should grow lush and thick. Best wishes with your lawn.
